Abstract Background Gastrointestinal Stromal Tumour (GIST) most commonly arises from the stomach or small intestine, and presents with mass effect gastrointestinal bleeding. In this unusual case, imaging demonstrated a double duct sign prompting pancreatic MDT referral. Case Presentation A 65-year-old gentleman background of rheumatoid arthritis COPD reported 2-month history anorexia one stone weight loss during rheumatology clinic follow-up. He was found to be anaemic but markers for his showed disease well controlled performed exclude malignancy. CT Thorax, Abdomen Pelvis revealed lung opacification, suggesting cancer metastasis. immediately referred Multi-Disciplinary Team underwent an Endoscopic Ultrasound which 12cm cyst extending pelvis gastric body. Biopsy confirmed that large thick-walled (GIST). Staging PET scan negative metastatic disease. laparotomy GIST excision cuff normal removed. Post-operatively, recovery prolonged due exacerbation however he eventually recovered discharged on day 11. There were no other post operative complications. Conclusions are rare tumours, making up less than 1% all tumour. Patients GISTs primary localised can achieve curative outcome through surgery alone. The prognosis tumours is favourable, 5-year survival rates at 83%. This case demonstrates value pre-operative endoscopic ultrasound.
